You will be required to attach a complete copy of the Order you received. Acceptable file formats for attachments are .jpg, .png, .gif, .doc, .xls, .pdf, .docx, .xlsx, .tiff, .raw, .jpeg.

If you are requesting a Fire Marshal Review for more than one Order, you must complete and submit a separate form for each Order.
Other supporting information is NOT required at this time. Instructions and guidelines about supporting information will be sent to you when your request is processed.

The Office of the Fire Marshal collects the personal information requested on this form pursuant to section 25 of the Fire Protection and Prevention Act, 1997. This information is being collected for the purpose of undertaking a Fire Marshal Review of an Order issued by an inspector and may be shared with other parties to the Order.
